This document explains how to access a STARS patient’s emergency plan when they arrive at an Emergency Department. Staff should go to <strong>www.starsprogram.org</strong> and log in using their department’s STARS email login (e.g., <strong>GlennonED@starsprogram.org</strong>). Patients are organized by <strong>STARS number</strong>. After logging in, the patient may already appear on the user’s dashboard; if not, staff should search by selecting <strong>“All STARS”</strong> and entering the <strong>STARS #</strong> or the <strong>patient’s name</strong> in the search field.

If staff are unable to log in to the STARS system, they should contact the <strong>Cardinal Glennon Access Center</strong> to request a copy of the patient’s plan. The caller should be ready to provide an <strong>email address or fax number</strong> so the Access Center can send the plan as a <strong>PDF</strong>.
